Plan of Action Services provides services in accordance with the Disability Act 2006 which establishes a contemporary legal framework for a whole-of-government and whole-of-community approach by reducing barriers and enabling people with a disability to more actively participate in community life. It creates a strong foundation for the provision and regulation of high quality supports for people with a disability and it safeguards and strengthens the rights of people accessing those supports.

Specifically, the operations of Plan of Action Services are guided by:
A. Quality Framework for Disability Service Providers
B. The Victorian Disability Industry Standards
C. The Victorian Disability Outcome Standards
D. Individualised Planning (Section 52 of the Disability Act 2006)

Plan of Action Services can provide

• Individualised planning for people with a disability
• Implementation of approved individualised plans
• Complementary individualised service (case management) where appropriate
• Individual, family and staff training in person centred approach
• Program assessments in line with DHS Quality Framework (eg. Outreach)
• Advocacy support for community inclusion, leadership and participation
• Self advocacy and family led planning training for individuals and groups
• Futures for Young Adults facilitation of planning
• Facilitation of One Plan for an individual where multiple providers are involved

Plan of Action Services also caters for people without a disability who are seeking support to develop a Plan of Action and mentoring to achieve the goals of their plan.
